Release and Waiver of Liability
In consideration of the Ottawa Lacrosse Club accepting this application, I hereby acknowledge for myself, my heirs, executors, administrators and assigns release the Ottawa Lacrosse Club, its respective servants, agents, directors, members and officers from any claims, demands, damages, actions or causes of action arising out of or in consequence of any loss, injury or damage to my person or property incurred while attending at or participating in a lacrosse match, practice or event notwithstanding any such loss, injury or damage may have risen by reason of the negligence or the Ottawa Lacrosse Club, its servants, agents, director, members or officers. Without limiting the generality of the foregoing, I further release any recourse, which I may now, or hereafter resulting from any decision of the Ottawa Lacrosse Club.
